Reinstatement Instructions

Attention Graduates: If you have NOT become a Member of the OAMRT/CAMRT since graduating, a reinstatement is NOT necessary.
Please contact the OAMRT Head Office for your pro-rated dues at 1-800-387-4674 Ext. 109


Complete an Application Form for Reinstatement.

Pay the Reinstatement Fees. (Awaiting CAMRT Update)
Note: For those who resigned your fees are Pro-Rated.

If you have not been a Member of the CAMRT/OAMRT for more than five (5) years, you must show proof of competency (copy of current registration card with the CMRTO) and attach it to the Reinstatement Application Form.

Return the completed Application Form, Fee owing and any documentation required to:
